By submitting a form on lipedema.ua, I consent under the Law of Ukraine “On Personal Data Protection” and, where applicable, the GDPR to LIPEDEMA LLC (company code 45687449) processing my name, phone number, email, preferred contact method, message, technical data and referral data.
Purpose: to contact me, arrange a consultation, provide information about the clinic’s services and, if I become a patient, provide medical services and keep medical records.
Health information. If I complete the questionnaire, I separately and explicitly consent to the clinic’s doctors processing my answers about symptoms for a preliminary assessment and consultation. This data is kept under medical confidentiality.
Duration: until the purpose is achieved or I withdraw consent. I may withdraw consent at any time by emailing inbox.lipedema@gmail.com; withdrawal does not affect processing carried out before it.
